Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active New York State physical therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in Skilled Nursing Facility (SNF) setting required. This must include evaluation, plan of care development, skilled treatment, and documentation consistent with SNF standards.
• Demonstrated competency with common SNF equipment and procedures, such as mobility assistance, transfers, gait training, therapeutic exercise, and use of assistive devices.
• Ability to collaborate with interdisciplinary teams, complete resident assessments, and meet regulatory documentation requirements.
• Reliable professional references documenting recent SNF experience preferred.
