Key Responsibilities

• Conduct thorough evaluations of each resident's physical capabilities to accurately assess their treatment needs and develop targeted plans.
• Create and implement individualized treatment plans that focus on enhancing residents' strength, mobility, and functional independence through evidence-based practices.
• Administer a variety of therapeutic interventions, including manual therapy, therapeutic exercises, and modalities, carefully adjusted to suit each patient's condition.
• Regularly monitor and document residents' progress, ensuring treatment plans are modified as needed to optimize recovery and well-being.
• Collaborate closely with a multidisciplinary team, including physicians, nurses, and occupational therapists, to provide holistic and comprehensive care to residents.
• Educate residents and their families on effective home exercise strategies, ensuring they are equipped to maintain and enhance their physical health post-therapy.
• Uphold compliance with all state and federal regulations as well as professional standards, ensuring the highest quality of care is always delivered.

Patient Population / Client Focus

This role primarily involves working with geriatric patients, providing them specialized physical therapy services tailored to their unique needs. You will enhance the quality of life for elderly individuals by helping them overcome physical impairments due to age-related conditions or recovery from surgery or illness. By delivering compassionate and focused care, you will play a vital role in improving their independence and mobility.

Qualifications

• A valid and current licensure as a Physical Therapist in New York State.
• A Bachelor's degree in Physical Therapy; a Master's degree or Doctorate in Physical Therapy from an accredited institution is preferred.
• Demonstrated strong communication, organizational, and interpersonal skills are essential for effective patient interaction and team collaboration.
• A commitment to maintaining the highest standards of care and an enthusiasm for ongoing professional development.
• Must be able to adapt to various home environments and deliver personalized therapy effectively.

Preferred Experience

• Experience working in home health or skilled nursing facilities is highly desirable.
