Key Responsibilities
- Evaluate patient conditions and create personalized treatment plans that reflect their unique needs and rehabilitation goals.
- Provide therapeutic interventions for an average of seven patients daily, including guiding them through mobility exercises, gait training, and functional activities.
- Utilize evidence-based practices and therapeutic methodologies to enhance patient recovery and improve overall functional outcomes.
- Engage in clear and supportive communication with patients and their families, educating them about treatment plans and progress, thus fostering a collaborative care environment.
- Maintain accurate and up-to-date patient records, tracking progress and outcomes while collaborating with interdisciplinary team members to optimize care.
- Participate in quality improvement initiatives aimed at enhancing the patient experience and organizational effectiveness.
Patient Population / Client Focus

Qualifications
- Current and valid Physical Therapist license issued by the New York State.

Compensation and Benefits
