Work Environment
The role requires working primarily in home care settings, where you will provide personalized therapy services directly within the community. This onsite work environment emphasizes direct engagement with patients in their familiar surroundings, allowing for tailored treatment plans that cater to individual needs. You will collaborate with a multidisciplinary team comprised of nurses, occupational therapists, and other healthcare professionals to create holistic care plans specific to each patient's requirements.
Key Responsibilities
- Evaluate patient conditions and design custom therapy plans aimed at improving strength and mobility.
- Implement evidence-based treatment modalities that align with best practices in physical therapy.
- Work directly with up to 7 patients per shift, delivering focused and attentive care.
- Educate patients and their families about therapy techniques, expected outcomes, and ongoing care strategies.
- Maintain accurate documentation of patient progress and treatment interventions following regulatory guidelines.
- Actively contribute to team meetings to discuss treatment plans and share insights on patient care.
- Foster a compassionate and motivational environment, encouraging patients throughout their rehabilitation journey.
Patient Population / Client Focus

Qualifications
- Must possess a valid and active Physical Therapist License in the State of New York.

- Experience utilizing patient management systems and electronic medical records for documentation.

Compensation and Benefits
