Genesis Home Health is seeking a compassionate, skilled Physical Therapist (PT) to join our growing home health team. We are a locally owned agency dedicated to helping patients maintain independence and heal safely in the comfort of their homes.
The Physical Therapist is responsible for evaluating, planning, and implementing individualized therapy programs for home health patients in accordance with physician orders and CMS Conditions of Participation.
This role focuses on improving mobility, strength, safety, and functional independence while reducing fall risk and rehospitalizations.
Requirements
Responsibilities: Perform initial evaluations and reassessments in patient homes Develop individualized Plan of Care in collaboration with physician Establish measurable functional goals Provide therapeutic exercises, gait training, balance training, and safety education Perform OASIS assessments as applicable Supervise Physical Therapist Assistants (PTA) as required Educate patients and caregivers on fall prevention and home safety Complete accurate and timely documentation in EMR Participate in care conferences and interdisciplinary collaboration
Qualifications Current Oklahoma Physical Therapist license Graduate of an accredited Physical Therapy program Minimum 1 year clinical experience (home health preferred) Strong knowledge of Medicare home health guidelines Valid driver’s license and reliable transportation Ability to work independently and manage schedule efficiently
Preferred Experience Home Health OASIS experience Experience with geriatric population Knowledge of CMS documentation standards
